The Wellness Connection is an innovative program that offers health and wellness resources in accessible locations tailored to the health needs of the community. The program provides education that allows community members to learn how to self-manage their health.
Each Wellness Connection site contains a blood pressure machine and a medical grade weight scale. Evidence suggests that by providing community members with the appropriate tools and resources, they will be better advocates for their health, which will help reduce their risk of chronic disease.
