The Community Health Strategy & Innovation team finds new ways to educate our community to promote long-term solutions to maintaining a healthier lifestyle. Using the Wellness Connection model, our community health nurse delivers preventive health initiatives directly to the underserved. Additionally, we provide evidence-based, sustainable programming to community members designed to overcome social determinants regarding health education and access to care.
Your Health is Your Wealth (YHYW) is an 8 to 12 module curriculum created by the CHSI team to supplement high school health class. In collaboration with the Kansas City Public School District (KCPS), YHYW facilitators provide real-world experiences and a hands-on teaching style that engages and inspires high school students to adopt and maintain healthy habits.
Since 2019 the CSHI team has been able to share the YHYW curriculum with about 160 students per school year (with the exception of the COVID-19 pandemic). Each year students share positive feedback regarding their YHYW experience and in 6-8 weeks students show a significant increase in understanding the importance of adopting healthy behaviors. On average students at Lincoln High School score 60% on their pre-health literacy assessment and 85% on the post health literacy assessment that students are required to complete at the end of the semester.
