Birth Certificates

To obtain your child’s birth certificate, click here

Birth certificates may be picked up at the Jackson County Public Health office in Lee’s Summit or at the Kansas City Health Department in KCMO.

The hospital does not issue birth certificates.

• If the father’s name is not on the birth certificate, print this form and mail it to MO Bureau of Vital Records. Please call 1-888-677-2083 if you have questions.
  
  
• If a name is misspelled, please call us at 816-404-5236. Most corrections may be completed over the phone.
  
  
• If you have received paperwork from the state informing you of an issue with your child’s birth certificate, please call 816-404-5236. We can solve most issues over the phone with the information from the state.

We accept walk-ins for birth certificate issues from 11:00 AM – 1:00 PM Monday-Friday.
