Patient Referrals

To refer a patient, please choose one of the following options:

Option 1: Enroll in the Provider Hub portal to enter referrals AND retrieve results electronically. This is the most efficient & secure option.

  • To enroll in the Provider Hub portal, please click here to download and complete the user request form.
  • Email the completed form to Provider-Hub@uhkc.org.
  • Once completed, your site administrator will receive an email with usernames and temporary passwords for your group.

If you are already enrolled in the UH Provider Hub portal, please click the Refer a Patient button.

Option 2: All other referrals may be submitted by clicking the REFERRAL FORM button. Note, the processing time for these will take longer than if you were to enter referrals directly into the UH Provider Hub. Self-referrals entered by patients will NOT be accepted.

Accessing the UH Provider Hub is easy and enables you to:

  • Enter referrals and orders that go directly to the clinic - no more waiting for faxes to be processed.
  • Check the status of your referrals without having to place a call.
  • Retrieve results electronically, alleviating the need to check for faxes.
